Hard-Down MRI Faults

If the console will not boot at all, or reports TPS Reset Failed or Host Monitor not running, start with GE Signa troubleshooting or MRI computer issues. Protocol and reconstruction failures are on MRI software issues.

Multicoil TR Bias Fault
The RF coil chain fails its bias check during Auto Pre-Scan and the system refuses to scan. More than one part of the chain can raise it, so it is diagnosed, not swapped.
Gradient Overcurrent or Gradient Not Ready
The gradient subsystem trips on overcurrent or never confirms ready after a recovery cycle. Until it clears, no sequence runs.
Scan stops mid-series
The table or scan halts with a patient in the bore and the study has to restart. It repeats, usually at a worse time, until the cause is found.
Patient Comfort Sensor trip
A bore-environment fail-safe stops the scan until conditions inside the bore are right. Nuisance trips point to a sensor, fan or airflow fault worth clearing before the next patient.

The Warnings Worth Acting On

A magnet also warns you on the cryogen side: a helium level that keeps trending down, or a cold head that starts knocking. Those live on our helium fill and cold head replacement pages. Image-quality drift with no fault message usually belongs on MRI calibration.

Transmit gain climbing at prescan
Prescan needs more RF drive every month to hit the same flip angle. That trend is an RF amplifier or coil path losing output before it fails outright.
Receive coil elements dropping out
One channel goes quiet and the image shows shading or a step in signal across the field of view. Coil faults grow from one element to the whole array.
Spike and zipper artifacts
Bright specks or a zipper line that follows a particular sequence. Gradient spiking and RF leakage both leave this signature and both get worse.
Gradient cabinet temperature warnings
The gradient amplifier or coil runs warm and the system throttles or trips on long sequences. It is a cooling or amplifier problem, and it reaches the cryogens if the chiller is shared.

How the Job Runs

Pull and read the full system log, not just the headline error. Isolate the failing subsystem, RF, gradient, table, coil or cryogen, confirm it, then repair, calibrate and verify before the room goes back.

It means the gradient subsystem is not confirming ready-to-scan status, and the cause can sit in more than one place in that chain. Replacing the wrong part is an expensive way to find out you guessed wrong, so we identify the actual fault before anything is swapped.

Almost any subsystem, coil, RF or gradient, can trigger it, which makes it one of the hardest faults to self-diagnose. The fix depends entirely on which one it is.

Mid-scan interruptions can come from several places in the system, and letting it go usually means it happens again, often at a worse time. Getting ahead of it keeps your schedule intact.
